If you want to be COMPLETELY realisitic there are alot of things that should be different.
Picard should be an Admiral 3 times over.
Riker should be the captain of the Enterprise.
Data should be the Captain of his own ship.
Laforge should be the Captain of his own ship.
Crusher could have her own (hospital)ship.
Wesley shoulda had a finger stuck in his eye.

Most of these things are due to the grevious losses the Federation has suffered from the Borg and Dominion.
Wesley should have a finger in his eye just because.
